Brief Description Case 1: Transverse myelitis in SLE
A 23 years old male with abdominal pain with tight band sensation for about 2 weeks. The general examination was normal, no epigastric pain. On neurological examination we found normal visual acuity, visual field, and fundus ODS. Mild weakness on both legs was noted after squatting and tiptoe walking. Hofman-Tromner reflexes were positive bilaterally (left is more severe). Bilateral finger escape test (+). Lhermitte sign (-). Bilateral APR-KPR 3+ (normal 2+). No segmental or dermatomal exteroceptive dysfunction pattern was found. Perspiration test was normal. Bowel & bladder functions were normal.
Routine haematology test and BSR and routine CSF findings. TORCH and tumor markers panel were unremarkable except for positive CMV IgG.
First spinal MRI was consistent with transverse myelitis with multiple lesions on cervical to upper thoracal segments.
He was treated with MP 1g IV for 4 days and pregabaline 50mg oral tid for 1 week. There was a significant improvement on the pain and tight band sensastion. However, follow up MRI two weeks later still showed no significant improvement. The MP was shifted to 8mg oral tid and pregabaline 50mg tid. The ANA profile result was consistent with SLE (Anti-Sm ++).
The patient was consulted to the Rheumatologist. Initial examination showed no other abnormality and the MP was changed to mycophenolate. However, since the pain was aggravated, the Rheumatologist decided to re-administer the oral MP.
